个人参赛声明书 Responsibility Letter

• 一.本人自愿参加2023七彩云南格兰芬多自行车节及一切相关活动(以下统称“比赛”)，本人具有参加本次比赛相应的民事行为能力和民事责任能力，全面理解并同意遵守组委会及协办机构(以下统称“主承办”)所制订的各项规程、规则、规定、要求及采取的措施。
• 二.本人明确了解参加本次对健康状况的要求以及比赛可能发生的一切风险，承诺已通过正规医疗机构进行体检，并确认自己的身体状况能够适应于本次比赛。为参加比赛已做好充分训练和准备,并经国家认证的医疗机构体检。本人愿意承担自身的意外风险责任，并确认不会就活动中发生或引发的自身意外、死亡或任何形式的损失向组委会提出赔偿或追究责任。
• 三.本人将向主承办提供身份证件用于核实本人身份及参赛资格，保证提交的身份证件和文件资料真实有效，并承担因提供不实信息所产生的全部责任。
• 四.本人同意在参赛过程中遵守裁判、医疗人员和安保人员的要求，在未完成本次比赛、身体不适及出现突发状况时主动退出本次比赛，并承担因本人坚持本次比赛所产生的全部责任与后果；同意接受主承办在本次比赛期间提供的现场急救性质的医务治疗，但在医院救治等发生的相关费用由本人自理。
• 五.本人同意主承办以本人为被保险人投保人身意外险，确认已知悉并同意保险合同的相关内容。
• 六.本人授权组委会及指定媒体无偿使用本人的姓名、肖像、声音和其它个人资料用于比赛的组织和推广。
• 1. I voluntarily participate in the 2023 Colourful Cycling Festival  (here-in-after referred to as the'Race'), and I have the corresponding civil capacity and civilliability capacity to participate in the race. Fully understand and agree tocomply with all procedures, rules, regulations, requirements and actions formulated by the OC and its co-organizers (collectively referred to as the'Main Organizer').
• 2. I clearly understand all risks that may occur during my participation in the raceand agree to bear all risks and responsibilities that may occur during my participation in the race ; I promise that I have passed a physical examinationin a formal medical institution and have confirmed that my physical conditioncan adapt to this race.
• 3. I will provide my identity certificate to the organizer for verification of my identity and qualification, guarantee the authenticity and validity of the submitted identity certificate and documents, and assume all responsibilities arising from providing false information.
• 4. I agree to abide by the requirements of the judges, medical personnel and security personnel during the race. I will voluntarily withdraw from the race  if I am not finished, I feel ill or have an emergency, and I will assume all responsibilities and consequences caused by my adherence to the race .I agree to accept the on-site emergency medical treatment provided by the main contractor during the race, but I shall bear the expenses related to hospital treatment.
• 5. I agree to undertake the personal accident insurance on my behalf and confirm that I have known and agreed to the relevant contents of the insurance contract.
• 6 .I authorize the Organizing Committee and the designated media to use my name, portrait, voice and other personal data for the organization and promotion of the race.

说明:

酒店入住时间：11月4日下午2点之后；离店时间:11月11日下午2点之前。

参赛旅行套包的区别主要在于酒店不同档次，详情请见后期赛事手册中后勤信息模块。

购买城市间转场交通服务的选手，可携带一辆自行车。

注意：

如选手选择单间服务的产品,组委会尽量满足大床房需求，在个别城市由于酒店硬件条件限制，可能会随机调整为标间。如介意，请慎选。

L型套包酒店距离赛事起终点都比较远，其中：玉溪澄江站选择的酒店为抚仙湖希尔顿酒店，距离赛事起终点15公里。如介意请慎选。

Note:

If registrants choose the single room service (A1/B1/C1), they will have a King-size bed room as priority choice. Due to the hardware constraints of the hotels in certain cities, king-size bed room maybe randomly adjusted to a standard room.
